I hereby consent to allow, through its designated agent and its employees to obtain and verify my consumer information (including credit, (Credit Report, Fico Score) criminal, housing records and public records information) for the purpose of determining whether or not to lease an apartment to me. I understand that a review of my credit history will result in a hard inquiry on my report. I understand that should I lease an apartment, its agent shall have a continuing right to review my consumer information, rental application, payment history and occupancy history for account review purposes and for improving application methods. I understand that the application fee is non refundable even if the application is denied. I understand and accept that by submitting this application, I have received the service of applying for the desired apartment regardless of the outcome of this application. I further understand and agree that even if I am denied the apartment or change my mind for any reason not to move forward with the application process, any payments submitted for the purpose of this rental application will not be refunded in any way. Any deposit, security or rent money is non-refundable, according to the conditions of the company I am applying with. I understand that providing any false statements will result in an immediate denial.

Code § 20-207 et seq., WeiMark is required to provide you with the following disclosure: The application information you are about to enter will be used by this community's owner, property manager, real estate broker or designated agent (the "Owner") to procure a tenant screening report on you, and the Owner will use this report to determine your suitability for housing. If your application is denied or other adverse action is taken against you due to the tenant screening report, the Owner must tell you so, and how to contact the screening company to obtain a free copy of the report. You may dispute inaccurate or incorrect information on the report directly with the screening company. The Owner’s screening company is Weimark and can be contacted at 877-Weimark. Annually, you may order a free report from www.annualcreditreport.com, in addition to a free report from each national consumer reporting agency if adverse action was taken against you.
GUARANTOR STATEMENT:

I am completing this application for the express purpose of acting as a Guarantor to the applicant of this
rental, and I have no intention of occupying the dwelling unit that is being applied for. I promise to
guarantee the tenants, in the even that the tenants fail to pay, I understand I may be required to cover
rent, and all required fees and charges. I hereby consent to allow, through its designated agent and its
employees to obtain and verify my consumer information (including credit, (Credit Report, Fico Score)
criminal, housing records and public records information) for the purpose of determining whether or not to
lease an apartment to the applicant for which I am acting as Guarantor. I understand that a review of my
credit history will result in a hard inquiry on my report. I understand that should the applicant for which
I am acting as Guarantor lease an apartment, its agent shall have a continuing right to review my consumer
information, rental application, payment history and occupancy history for account review purposes and for
improving application methods. I understand that the application fee is non refundable even if the
